1. Use of the Just Culture Community Behavioral Benchmark Survey is granted through this license agreement.
2. Purchase of one credit ($500) allows a user to create one survey instrument for one identified value (e.g., employee or customer safety) with a maximum number of survey respondents of 500,000.
3. Users will be granted access to the administrative site for a period of three years. Users may regenerate the report to benchmark their data against a larger population of organizations that participate; for a period of one year. Users will not be able to re-open the survey itself once the user has closed the survey.
4. User agrees to allow Outcome Engineering to occasionally change the form of specific survey questions as a means of researching biases associated with a particular question. We will do this on no more than 1% of individual survey takers, and when we do, that specific survey will not be included in the overall results reported back to the user.
5. Outcome Engineering will not make public any specific data tied to an identifiable organization. Outcome Engineering may use collected data as a research mechanism within the Just Culture community, and will reserve rights to publish or report out data in a manner that does not disclose any individual organization, implicitly or explicitly.
6. Methods and materials provided in this tool are copyrighted, and may only be reproduced for internal use within the participating organization.
